What Does Medicare Not Cover? Key Gaps to Know in 2026

Medicare covers a lot — but there are meaningful gaps that surprise many new enrollees. Knowing what is excluded helps you plan ahead and choose the right supplemental coverage.

Routine Dental, Vision, and Hearing

Original Medicare does not cover routine dental cleanings, eye exams for glasses, or hearing aids. Some Medicare Advantage plans include limited dental, vision, and hearing benefits — one reason to compare them carefully.

Long-Term Care

Medicare does not pay for custodial care — help with bathing, dressing, and daily activities in a nursing home or at home. It only covers short-term skilled nursing stays after a qualifying hospital admission. Long-term care insurance or Medicaid may fill this gap.

Overseas Care

Medicare generally does not cover healthcare outside the United States. Some Medigap plans (G, D, M, N) include limited foreign travel emergency coverage up to plan limits.

Cost-Sharing Under Original Medicare

In 2026, the Part A hospital deductible is $1,676 per benefit period. Part B requires 20% coinsurance after your deductible — with no out-of-pocket cap under Original Medicare alone.

How to Fill the Gaps

A Medicare Supplement (Medigap) plan covers most cost-sharing. A Medicare Advantage plan caps your annual out-of-pocket costs. Neither covers long-term custodial care.
